JARDIN Care Pathways Toolkit

Many rare disease patient groups are proud members of European Reference Networks (ERNs).

ERNs are international specialised networks across Europe that focus on improving care for those living with rare or complex diseases.

Currently, there are 24 ERNs that support knowledge generation, professional training,  education and research.

These include ERNs, such as:

What is JARDIN?

Joint Action on Integration of ERNs into National Healthcare Systems (‘JARDIN’) aims to:

 

  • Integrate European Reference Networks (ERNs) into National Healthcare Systems
  • Improve the accessibility and support the long-term sustainability of the ERN system
  • Strengthen the resilience of national health systems

What is the JARDIN Care Pathways Toolkit?

​The JARDIN Care Pathways Toolkit is a guide designed to help healthcare professionals, managers, and patient representatives collaboratively design and develop the ERN care pathways being supported in the JARDIN Joint Action for individuals with rare diseases.

These pathways aim to ensure that rare patients receive coordinated, high-quality care across various healthcare services.

The Toolkit serves as a companion resource, outlining the essential steps, roles, and responsibilities involved in creating effective care pathways.

It emphasises a patient-centered approach, ensuring that care plans are tailored to individual needs and involve input from both medical experts and patients themselves.
