JARDIN Care Pathways Toolkit
Many rare disease patient groups are proud members of European Reference Networks (ERNs).
ERNs are international specialised networks across Europe that focus on improving care for those living with rare or complex diseases.
Currently, there are 24 ERNs that support knowledge generation, professional training, education and research.

What is JARDIN?
Joint Action on Integration of ERNs into National Healthcare Systems (‘JARDIN’) aims to:
- Integrate European Reference Networks (ERNs) into National Healthcare Systems
- Improve the accessibility and support the long-term sustainability of the ERN system
- Strengthen the resilience of national health systems
